I see lots and lots of medical staff on the train into South Station (Boston) - most of whom get off at Ruggles or Back Bay. I don't recall ever seeing any of them in a dress or skirt - they are all in scrubs. Same when I used to live NW of Boston and travelled in on the Red Line - lots of medical folk got off the train at Charles / MGH and all wore scrubs.

As an aside, I've always been surprised that medical staff are allowed to travel into work in their scrubs. I would have thought that wearing their work clothes on the commute would be a good way of bringing in germs and other nasties.

Mash was a parody of the Medical units of the Korean War and while some was actually very accurate most was just for the comedic value. They did have some very real footage of the real war and did some shows that told the truth about what it was like. I watched all of the shows at least twice and some many more than that. The last few years the show slowly became a political battle ground and did not truly meet the premise of the original show. It was nothing like the movie either.

In practise, it's vastly more complicated.
